With APA 7th, you use a colon to separate all of the titles and subtiltes, and use a capital letter at the start of each title element.
For example, a work with the following title/subtitle combination:
Moving Forward in Health
Closing the Gap in Rural and Remote Health Care
A Review of the Literature
would be formatted like this:
Moving forward in health: Closing the gap in rural and remote health care: A review of the literature.
